Predictive Modeling software helps predict a patient’s future expense as well as steer the clinicians to the right patients at the right time for the right intervention. The necessary information is provided to make the right diagnosis and develop the right treatment plan. Patient provided medical information and biometric readings from the Health Risk Assessment Process are then combined with on-site clinic encounter data, medical and pharmacy claims, and laboratory readings. Patients at probable risk of a catastrophic medical event can be predicted, and the on-site clinician can intervene many times before something occurs.

Through guideline gaps analysis of medical, pharmacy, clinic encounter and laboratory data, patients are identified who have missed important tests and this information is provided to the on-site clinician. The patient can then be counseled on the need for a test, which in many instances can be administered in the clinic.  If the test cannot be done in the clinic, the on-site clinician can also make the necessary referral to an appropriate provider.
